Reishi, ashwagandha and mint: the names on the label
GBIF records reishi as a common name of Ganoderma lucidum (Curtis) P.Karst., the lacquered bracket, an accepted species of the family Polyporaceae. Ashwagandha is listed there among the names of Withania somnifera (L.) Dunal, a plant of the nightshade family, Solanaceae, also called Indian ginseng or winter cherry. Mint is the common name of the genus Mentha in the mint family, Lamiaceae; the label does not say which mint species is used, so none is named here.
Bottle and box

Reishi, lion’s mane, chaga, cordyceps and maitake have a long history in Asia and Eastern Europe, both in cooking and in traditional medicine.
